Prior to commencement of mystery shopping services, Quality Track will confirm the total cost for each type of call service selected. All billing is quarterly in advance. Service will begin once payment has been received.

Subject to confirmation of total cost, I hereby authorize Quality Track International to conduct mystery calls as detailed on this form and acknowledge that hotel staff who handle these calls have been notified that their conversations with QTI mystery shoppers may be recorded for evaluation purposes.
